Coaches Medical Declaration Form

CR Form - Coach Registration Form for the level 1 Course, level 2 course and moderate water endorsement Send this off to your national body with your fee and 2 star and FSRT certificates (if you are doing your 2 star and or FSRT at the same time not this on your CR. It will come back stamped and with your candidate pack. This is £55 for non members or £39 for members. The £55 includes a years membership and insurance.
